I think the issue was by ths following , The first step is to match each label select option to a unique specific name, to be able to associate them in the following view.
Template
code :
{% for name in title %}
  <label> {{ name }} :</label>
  <select name="choice-{{ loop.index }}">
         <option disabled selected value> Select One</option>
         <option value="time">Temporal</option>
         <option value="single">Single Choice</option>
         <option value="multiple">Multiple Choice</option>
  </select><br>
{% endfor %}
@app.route("/config", methods=['POST'])
def config():

    choices = {}
    with open(request.form['filename'], 'r') as csvfile :
        reader = csv.reader(csvfile)
        titles = next(reader)
        for index, title in enumerate(titles,1):
            choice = 'choice-' + str(index)
            choices[title] = request.form.get(choice)

    #choices = {'Mode of Transport': 'time', 'Timestamp':'multiple' }
    choices = []
    with open(request.form['filename'], 'r') as csvfile :
        reader = csv.reader(csvfile)
        titles = next(reader)
        for index, title in enumerate(titles,1):
            choice = 'choice-' + str(index)
            choices.append((title, request.form.get(choice))

    #choices = [('Mode of Transport','time'), ('Timestamp','multiple')]

should help you out This turned out to be a result of the fact I was using {smarty} tags to build my php, the code was as written but only worked when it was all included in one smarty tag, I'm not sure I understand why that should be the case but in any regard it was fixed by including it all in one tag.

it fixes the issue You can use a combination of HTML and JavaScript (JSFIDDLE):
code :
<select id="opts" onchange="showForm()">
    <option value="0">Select Option</option>
    <option value="1">Option 1</option>
    <option value="2">Option 2</option>
</select>

<div id="f1" style="display:none">
    <form name="form1">
        <select id="opts" onchange="showForm()">
            <option value="0">Select Option</option>
            <option value="1">Option 1A</option>
            <option value="2">Option 1B</option>
        </select>
    </form>
</div>

<div id="f2" style="display:none">
    <form name="form2">
        <select id="opts" onchange="showForm()">
            <option value="0">Select Option</option>
            <option value="1">Option 2A</option>
            <option value="2">Option 2B</option>
        </select>
    </form>
</div>

<script type="text/javascript">
    function showForm() {
        var selopt = document.getElementById("opts").value;
        if (selopt == 1) {
            document.getElementById("f1").style.display = "block";
            document.getElementById("f2").style.display = "none";
        }
        if (selopt == 2) {
            document.getElementById("f2").style.display = "block";
            document.getElementById("f1").style.display = "none";
        }
        if (selopt == 0) {
            document.getElementById("f2").style.display = "none";
            document.getElementById("f1").style.display = "none";
        }
    }

code :
window.addEventListener('click', function(event) {
    var editMenuDropdowns = document.getElementsByClassName('edit-menu-drop-down-box');
    for (var j = 0; j < editMenuDropdowns.length; j++) {
      var openEditMenuDropdown = editMenuDropdowns[j];
      if (openEditMenuDropdown.classList.contains('show-edit-menu-drop-down-box') && event.target.closest('.edit-menu-icon-con') !== openEditMenuDropdown.parentElement) {
        openEditMenuDropdown.classList.remove('show-edit-menu-drop-down-box');
